You do not need to apply any creams, lotions, or ointments while breastfeeding. In fact you should wash with plain warm water and avoid any lotions all together. Soaps, perfumes, lotions and other ointments can mask your natural scent and interfere with breastfeeding. Baby's nursing reflexes are triggered by the smell of your milk. Additionally, soap can dry out your skin and cosmetics my not be safe for your baby to digest.
The only time I recommend using nipple cream is if your nipples are damaged. Nipple cream, in this case, can help sooth and heal the affected area. Look for a nipple ointment that contains only 100% pure lanolin (like Lansinoh or PureLan) and apply sparingly until healed. Alternatively you can apply a little breast milk and allow your nipples to air dry. Nipple damage is a sign that the baby is not well attached while feeding. A proper latch will prevent sore nipples. So, along with using a nipple cream you'll need to improve your baby's latch.
